To make the most of your clinic
visit, please bring the following personal
medical information with you to the
appointment:
- Completed medical history and new
problem form (if you completed this in
advance)

Check In
Take the main elevators to the 5th Floor
of the1200 Binz Medical Office Building. At
the reception desk, we will verify your
personal information, referral
authorization, and collect your co-pay. Here
you will be asked to fill out a medical
history form. To speed up this process, you
can download this form and fill it out prior
to your visit.
Dr. Smith’s Exam
Dr. Smith will take a thorough history
and perform a detailed examination. During
the visit, please feel free to ask
questions. After the exam, you will receive
a clear explanation of your diagnosis and
the reasons for any additional tests.
Treatment options will be discussed and we
encourage your participation in planning the
course of care.
